Hello, may I please speak with ___________? This is ___________ calling from the Gallup Organization in Lincoln, Nebraska. You were recently sent a letter from the Occupational Safety and Health Administration (OSHA) asking you to participate in an on-line questionnaire regarding work you do with the Voluntary Protection Programs. To date, our records indicate you have not completed the on-line questionnaire. Your responses are a vital part of this project. Do you need the website and access code again? [NOTE TO INTERVIEWERS: Some respondents have multiple sites that they will be filling out a questionnaire for. They are being asked to fill out one questionnaire PER SITE (so if they have three sites, they should fill out three questionnaires).
